CITY OF CRESTVIEW HILLS
RECREATION SURVEY

The City of Crestview Hills has been offering various recreation programs for all of its citizens.  Some of the programs have been well attended and some have not.  It is very important that we receive your input and views concerning both old and new programs so that we may offer our citizens quality programs throughout the year.  Please fill out the enclosed short survey and mail it to the City Building, or drop it off at 50 Crestview Hills Mall Road during regular business hours.  If you have any questions or comments, please call the City Building at 341-7373.
